How to Access Your Home Exercise Program
From the App
- Download the H2 Health app for your device (iOS or Android).
- Open the app.
- Enter your cell phone number.
- Enter the one-time passcode (OTP) you receive via text to verify your account.
- From the app's main menu, tap HOME EXERCISES, then START MY EXERCISES.
From the Web
- Go to the Online Portal: https://h2health.myhealthid.net/#/login
- Enter your cell phone number, then the one-time passcode (OTP) you receive via text to log in.
- Click the HOME EXERCISE PROGRAM button and enter your name, email, and date of birth.
- Select START EXERCISES to begin your program.

A home exercise program is a customized set of exercises your therapist gives you to do at home between your therapy visits. It’s designed to help you continue making progress, build strength, reduce pain, and improve mobility or function.
Therapy sessions alone aren’t enough to get the best results. Doing your HEP consistently helps reinforce what you do in therapy, speeds up recovery, and helps you return to your daily activities faster.
Some movement is better than none. If you're short on time, let your therapist know—they can help you prioritize the most important exercises so you still make progress.
It’s okay to take a rest day occasionally, especially if you’re feeling unusually sore or fatigued. But make sure to let your therapist know—they may need to adjust your program to better fit your recovery pace.
Yes! Even if you're feeling better, it's important to finish the full course of your HEP. This helps prevent re-injury and ensures long-term improvement. Your therapist will guide you on when and how to taper off the exercises.
You should begin to notice improvements in pain, mobility, strength, or function over time. Your therapist will regularly check your progress and update your HEP as needed to keep you moving forward.
It’s best to follow your therapist’s instructions closely. Adding exercises or doing more than prescribed can lead to overuse or injury. Always check with your therapist before making any changes.
