For Providers Looking to Refer
Are you looking to refer a patient and need paperwork to help get them started? Download our e-referral and send via your preferred method. We will contact the patient within 24 to 48 business hours to get them scheduled. We look forward to helping your patients!
Referring is easy.
- Email the completed referral to referrals@h2health.com
- Fax the referral to our central scheduling team at (904) 757-9679
- Find the patient's closest location here, and fax the referral directly.
